PRISON CHAPLAINS

• Please complete this form and send it to us, even if your facility issues a purchase order, because we need all of the information on this form to process your order.
  
• Along with this order form we need a letter from someone in authority on your institution stationery stating that the institution will accept the baptistry as a gift from American Rehabilitation Ministries and will be responsible for the shipping charges. If a water heater and/or casters are ordered, the institution must also confirm in writing that they will be responsible for their cost.
  
• The baptistry must be shipped to and placed in the correctional facility.
  
• The baptistry will be shipped freight prepaid on a commercial truck line. We will send you an invoice and a copy of the freight bill, and we expect to be reimbursed within 30 days of the invoice date.

MILITARY CHAPLAINS

• Please complete this form and send it directly to us. Even if a purchase order will be sent, we need a copy of this form to process your order.
  
• No deposit is required for military orders.

OVERSEAS MISSIONS

• Please complete this order form and return it to us with your deposit. When your order is received we will send you a confirmation letter and a projected shipping date.
  
• The balance due for your order is payable within 30 days after your baptistry is shipped. You will be invoiced at the time of shipment. You may pay the full amount of your deposit if you desire.
  
• Be sure to confirm all-shipping arrangements and include them with this order form.

CHURCHES

• Please compete this order form and return it to us with your deposit. When your order is received we will send you a confirmation letter and a projected shipping date.
  
• The balance due for your order is payable within 30 days after your baptistry is shipped. You will be invoiced at the time of shipment. You may pay the full amount of your deposit if you desire.
  
• We will notify you by telephone when your baptistry is shipped, and the trucking company should call you, for carrier's convenience, before delivery of your baptistry.
  
• The freight charges will be invoiced after shipping and are to be paid in full within 30 days from invoice date.
  
• Any damages should be reported directly to the trucking company and ARM as soon as possible.  We will be happy to provide any information or assistance if repairs are necessary.
  
• We must have the telephone number of the person who will need to be contacted by the trucking company to make delivery.
  
• If you are ordering only a fiberglass tub or water heater, the entire cost is payable in advance.
