Instructions for Ordering Communion Tables

• Please compete this order form and return it to us with your deposit.
  
• The balance due for your order is payable within 30 days after your Communion Table is shipped. You will be invoiced at the time of shipment. You may pay the full amount with your deposit if you desire.
  
• We will notify you by telephone when your Communion Table  is shipped, and the trucking company should call you, for carrier's convenience, before delivery of your baptistry.
  
• The freight charges will be invoiced after shipping and are to be paid in full within 30 days from invoice date.
  
• Any damages should be reported directly to the trucking company and ARM as soon as possible.  We will be happy to provide any information or assistance if repairs are necessary.
  
• We must have the telephone number of the person who will need to be contacted by the trucking company to make delivery.
